2. The discount prices are only available for appointments booked and completed before 30 April

3. Before any booking can be completed, we will need to ensure you are eligible for any aspect
of the appointment against our clinical referral criteria.
4. The discount price is only available against the appointment made at the original date/time of
booking. If the appointment is cancelled or moved, Lifescan reserves the right to withdraw
any discount.
5. All appointments must be paid for at the time of booking, or on the day of the appointment*
*Where the appointment takes place at a location other than one of the dedicated Lifescan Centres (Guildford and Manchester), payment must always be made in full before the appointment date.
