3. Before any booking can be completed, we will need to ensure you are eligible for any aspect
of the appointment against our clinical referral criteria.
4. The offer/discount price is only available against the appointment made at the original
date/time of booking. If the appointment is cancelled or moved, Lifescan reserves the right to
withdraw any discount.
5. All appointments must be paid for at the time of booking, or on the day of the
appointment*.
*Where the appointment takes place at a location other than the Lifescan
Centre at Guildford, payment must always be made in full before the
appointment date.
